About Harmony Health

Our Mission

 

Harmony Health advances integrated housing and whole-person care solutions designed to improve long-term outcomes for high-need populations in Delaware.

Through structured partnerships, campus-based development, and coordinated service delivery, we address both clinical needs and the social drivers that impact health stability.

Harmony Health is a 501(c)(3) nonprofit organization serving as the contracting and community development entity for integrated housing and whole-person care initiatives in Delaware. 

Harmony Health partners with MaxWellness MD Health as its clinical delivery partner. MaxWellness provides integrated medical and dental services aligned with Harmony Health’s housing and supportive care initiatives. 

 Harmony Health prioritizes:

• Veterans
• Individuals with Intellectual & Developmental Disabilities (IDD)
• Young adults transitioning from foster care
• High-need Medicaid populations

Harmony Health has secured a campus-style property and is advancing phased development aligned with state and community needs.
