Who We Are
We hold the mission, secure the contracts, and coordinate the care.

Harmony Health of Delaware is a 501(c)(3) nonprofit, the operating name of Word of Mouth Ministries, Inc. We serve as the contracting and community development entity for integrated housing and whole-person care in Delaware.

Through structured partnerships, campus-based development, and coordinated service delivery, we address both the clinical needs and the social drivers that decide whether health and housing actually hold. We secure the agreements, place the residents, and connect the services, while our clinical partner delivers the care.

Structured Housing

Safe, stable, supportive housing for people who are the hardest to place, developed in phases and aligned with state and community needs.

Whole-Person Care

Housing paired with integrated medical and dental care delivered by our clinical partner, MaxWellness, so health and housing move together rather than apart.

Care Coordination and SDOH

We coordinate the social drivers of health, food, transportation, benefits, and connection, that determine whether care and housing last.

Workforce Development

The MaxWellness Healthcare Academy trains the next generation of the healthcare workforce, staffing the very system our residents rely on.

Medically complex, high-need Medicaid populations who cost the system the most when care falls apart.

Housing and the Campus

A campus secured, and a plan moving forward.

Harmony Health has secured a campus-style property and is advancing phased development aligned with state and community needs. Rather than scattered beds, the model brings housing, care, and workforce training onto one connected campus, with scattered-site supportive homes reaching back into Delaware neighborhoods.

Under Delaware's DDDS provider pathway, Harmony Health is building the infrastructure to place and support adults with intellectual and developmental disabilities, backed by care coordination that follows the resident, not the building.
